NALU Evaluation Consent

Dear Parent/Guardian,

We invite your child to participate in an evaluation survey for the Pacific American Foundation's NALU Studies program. This survey serves two important purposes: to assess the program's impact on your child and to test a new evaluative tool designed for indigenous cultural STEM programs.

Survey Purpose

Evaluate the NALU Studies program's effectiveness in:

  • Strengthening your child's connection to people and place
  • Enhancing their understanding of ecological functionality
  • Test a new evaluative tool specifically created for indigenous cultural STEM programs

Survey Details

  • Format: Online questionnaire
  • Duration: Approximately 10-15 minutes
  • Administration: Post program participation

Confidentiality

Your child's responses will remain strictly confidential. We will not associate any identifying information with the survey responses, and we will analyze and report all data in aggregate form only.

Voluntary Participation

Participation is entirely voluntary. Your child may skip any questions or withdraw from the survey at any time without consequences.

Benefits

While there are no direct benefits to your child, their participation will:

  1. Help improve the NALU Studies program
  2. Contribute to the development of culturally responsive evaluation methods for indigenous STEM education

Risks

There are no known risks associated with this survey beyond those encountered in daily life.

New Evaluative Tool

The survey incorporates a new tool designed to assess indigenous cultural STEM programs more effectively. This tool aims to:

  • Assess the integration of traditional knowledge in STEM education
